Referrals/Precerts
What is a referral? A referral is documentation in form of a script, verbal referral (documented in chart), or an insurance form from a primary care physician stating the need for patient to see a specialist.

What is a precert? A precert is documentation that you have gotten prior insurance approval before a test. Precerts are completed by Ohio Heart on your behalf.

If you are unsure if your plan requires a referral or precert to see a specialist, please contact your insurance carrier for your coverage requirements.

Co-Payments
What is a co-payment?
Co-payments are payments for services not covered by an insurance company are due at the time of service. Please remember to bring your insurance card, Medicare or Medicaid card with you to your office visits. For your convenience, Ohio Heart accepts cash, personal checks, Visa and Mastercard.

No Insurance/Financial Assistance
What if I do not have insurance? If you do not have health insurance, we ask that you pay in full at time of service. When your appointment is scheduled, you will be informed of the amount that will be due at your visit. If you are unable to pay in full, the billing office will contact you to set up payment arrangements and/or financial assistance.

Statements
Once your charges have been billed and benefits determined by your insurance plan, you will receive a statement from Ohio Heart detailing your account activity and financial responsibility, if any. Statements are sent out about every 38 days.
